In the U.S., egg producers can call their products "free range" even if the hens are in an industrial warehouse with one, small open door most cannot access, and that is for the vast majority of "cage-free" or "free range" birds. There is also no required oversight as to the quality of the external environment, the amount of time a hen would have access to it, or even if accessing it is possible.
